Home Health is offered within the hospital setting through referrals from the patient's physician. Private insurance patients need to be sure their insurance covers the home health services being rendered. Services offered include skilled nurses who assess and instruct the patient regarding disease processes, medication management, ordered treatments, wound care, injections, IV therapy, fall assessments and prevention management, and post-surgical needs.

Physical therapists can provide gait training, muscle strengthening, and personalized home exercise programs. Home health aides can provide personal care such as assistance with bathing, dressing, and taking vital signs to name a few. Social workers assist with connections to community services, financial concerns, transportation needs, and assistance with obtaining ordered medications.
